You save the configuration on a router with the copy running-config startup-config command and reboot the router. The router, however, comes up with a blank configuration. What can the problem be?

Correct answer: The configuration register setting is incorrect.
A blank configuration after a successful save points at the boot field: with the register at 0x2142 the router deliberately ignores NVRAM, so the saved configuration is present but never read. It is almost always the residue of a password recovery in which the register was not returned to 0x2102. show version confirms it in one line.
